A baby received ten times too high dose

In February this year, a premature baby received a ten-fold high dose of a blood-thinning medicine. Now the Inspectorate for Health and Care Services, IVO, has completed the investigation after the incident, writes Smålandsposten.
The reason of the matter was unclear instructions, insufficient knowledge and habit of nurses, writes Smålandsposten. The staff forgott that the dose needs to be mixed.
